Corpus Christi Police responded to the 8000 block of Yorktown Boulevard after a man that was walking in the roadway was hit by a vehicle.

When Officers arrived they found that the 30-year-old pedestrian had been struck by the 2013 Chevrolet truck had been killed.

Traffic Officers responded and the crash investigation revealed that the man was walking east on Yorktown Boulevard when the truck, also traveling east, struck him from behind and killed him.  The driver of the truck told Investigators that he did not see the man in the roadway until it was too late. Other witnesses also told Officers that they saw the man dart into the path of other vehicles before he was hit.

While investigating this case, Officers determined that the driver of the truck, Joseph Palmatier (01/27/1982), was under the influence of marijuana. He was arrested and charged with driving while intoxicated.

At this time, Traffic Officers continue to investigate this case and the driver has not been charged with any crimes in regards to the crash with the pedestrian.

NEW YORK, N.Y. – 12/05/2015 — Manhattan District Attorney Cyrus R. Vance, Jr., today announced the guilty plea of STELLA MEDNIK, 35, for crashing a car in Herald Square last December while under the influence of drugs and alcohol, seriously injuring pedestrians. MEDNIK pleaded guilty to the top charges of the two indictments against her, including Assault and Vehicular Assault in the Second Degrees, as well as Aggravated Unlicensed Operation of a Motor Vehicle in the First Degree, among other charges. She is expected to be sentenced on December 21, 2015.

“As a former lawyer, this defendant’s blatant disregard for not only our City’s laws but public safety, is particularly egregious,” said District Attorney Vance. “Pedestrians, cyclists, and drivers alike were endangered when Stella Mednik took the wheel in an extremely intoxicated state. Miraculously, no one was killed, but five innocent pedestrians were injured, including multiple with broken bones, by Ms. Mednik’s extraordinarily reckless decision to drive while under the influence of alcohol and crack cocaine.”

According to the defendant’s guilty plea and statements made on the record in court, at approximately 10:00 p.m. on December 10, 2014, MEDNIK crashed into a vehicle while traveling eastbound on West 34th Street near Sixth Avenue while impaired by alcohol and crack cocaine. Attempting to leave the scene, MEDNIK drove the wrong direction on West 34th Street and crossed Sixth Avenue, before attempting to re-enter the eastbound lane. MEDNIK hit a second vehicle and then drove onto the sidewalk, where she struck five pedestrians, pinning one woman against a building. Two of the victims suffered broken bones, and others were treated for back and neck pain. Two pipes with crack cocaine residue were recovered from MEDNIK’s car by NYPD officers. At the time of the crash, MEDNIK was driving with a suspended license.

Troopers Investigating Serious Motor Vehicle Crash, Hockessin

Hockessin – 12/05/2015- The Delaware State Police continue to investigate a serious motor vehicle crash which occurred on Lancaster Pike in Hockessin on Dec. 3, 2015.

The investigation has determined that the crash occurred at approximately 6:05 p.m., as Jose L. Escobar, 21, of Kennett Square, PA., was operating a 2003 Mitsubishi and was traveling in a southbound direction on Lancaster Pike (SR41) approaching the intersection of Ace Memorial Drive, Hockessin. The front of the Mitsubishi then struck the rear of a 2000 Nissan Altima, which was stopped on southbound Lancaster Pike, and which was being operated by Francisco Valadez, 31, of Kennett Square, PA. It was at this point that the rear of the Mitsubishi was then stuck by the front of a 1994 Ford Explorer which was also traveling southbound on Lancaster Pike, and which was being operated by Jose Vega, 55, of Hockessin, DE. The Mitsubishi was then forced into the northbound lane of Lancaster Pike where it was subsequently struck by a 2015 Volvo tractor and trailer, which was being operated by Dwayne Schuler, 50, of Nottingham, PA.

Francisco Valdez was removed from the scene by Emergency Medical Services and transported to the Christiana Hospital Trauma Center where he was treated for his non-life threatening injuries. It is unknown if Valdez was properly restrained at the time of the crash.

Jose Escobar was also removed from the scene by EMS and transported to the Christiana Hospital where he was also treated for non-life threatening injuries. Escobar, who was properly restrained at the time of the crash, was cited by troopers for Following Too Closely.

Jose Vega, who suffered serious injuries in the crash, was treated at the scene by New Castle County Paramedics and then transported to the Christiana Hospital where he was admitted. Troopers have obtained warrants charging Vega with Vehicular Assault 1st Degree, Driving under the Influence of Alcohol, Driving without an Operator’s License, No Motor Vehicle Insurance, and several other traffic offenses. Vega will be formally arrested and arraigned upon his release from the hospital. It is unknown if he was properly restrained at the time of the crash.

Dwayne Schuler was not injured in the crash.

Both the northbound and southbound lane of Lancaster Pike between Valley Road and the Pennsylvania state line were closed as the crash was investigated and cleared.

ROCKFORD, ILL.  – 12/05/2015 – The Rockford Police Department says they have charged a driver with DUI after a deadly crash that killed one person on the Whitman Street bridge early Friday morning.

Police say a Nissan Xterra, driven by James Allen Jeffers, 33, of South Beloit, was traveling westbound on the bridge, crossed the median, and crashed head-on into a Chevy Malibu, driven by Alex Banks, 25, of Rockford. A passenger was also in the Malibu, Tanisha Gates, 23, of Rockford.

Banks was pronounced dead at the scene of the crash, and Jeffers and Gates were transported to a local hospital in critical condition.

Police have charged Jeffers with driving under the influence of alcohol and improper lane usage. Officials say more charges are possible.

At approximately 1:25 a.m on 12-04-2015, members of the Rockford Police Department responded to the east side of the Whitman St Bridge, to investigate a rollover traffic crash.  Upon arrival, officers located two vehicles that had been involved in a head-on collision.  One of the vehicles that were involved had come to rest on its side.  Initial investigation revealed that one of the involved vehicles was traveling westbound in the eastbound lanes before the crash.

The 25-year-old driver of one of the vehicles sustained fatal injuries and was pronounced dead at the scene.  The passenger in that vehicle was a 23-year-old female, who sustained serious injuries, and was transported to a local hospital.  She is in serious condition.

The driver of the second vehicle also sustained serious injuries, and was transported to a local hospital for treatment. Police say that the crash is an on-going investigation.  Anyone who may have witnessed the crash is encouraged to contact the Rockford Police Department.

12/07/2015  — Garner Police arrested 22-year-old Javier Orlando Alvarez-Lopez of Wake County for the September 9th fatal crash at Whiteoak Road and Timber Drive East in Garner.  Alvarez-Lopez, pictured above, was charged with Driving While Impaired, Felony Death by Motor Vehicle, and Failing to Comply with License Restrictions.  He is currently in the Wake County Jail under 100,000 bond.

Garner police were dispatched to a crash at White Oak Rd. and Timber Dr. East at approximately 6:31 p.m. on September 9, 2015.

The preliminary investigation  indicates that a 2011 Cadillac SUV driven by Brenda Culbreth, age 69 of Garner, and a 2006 BMW sedan driven by Javier Orlando Alvarez Lopez, age 22 of Wake County, collided in the intersection.  Mrs. Culbreth was transported to WakeMed Raleigh where she died.  Lopez, as well as his passengers, Jose Francisco Torres, age 23 of Wake County, and Agustin Alejo Pascual, age 21 of Wake County, were also transported to WakeMed Raleigh.  All three occupants of the BMW sustained serious injuries.

The Garner Police Department’s Crash Reconstruction Team, as well as members of GPD’s Investigative Division, responded to investigate the cause and circumstances surrounding the crash.  At this time, no charges have been filed, and the crash remains an active investigation.  Anyone who witnessed the crash or was in the area of White Oak Rd./Timber Dr. E. with information is asked to contact the Garner Police Department at 919-772-8810.

Washington State Patrol reports that one person is dead as a result of a driver suspected of driving under the influence crossing the center line and hitting other vehicles.

On Dec. 6, 2015 at 12:06 pm Trooper S. Gustafson reports that in Skagit County on Francis Road, two miles northeast of Mount Vernon, a five-car crash was caused by Michael J. Stephenson, 43, of Mount Vernon, Washington, who was jailed in the Skagit County Jail on charges of vehicular homicide and assault. Stephenson was operating a 2005 Chevy Silverado which crossed the center line of the highway.

 

 

Lucia Morales, 28, of Mount Vernon, was driving a 1998 Ford Ranger and was not injured.

Jill S. Elwick, 49, of Sedro Woolley, Wa., was the driver of a 1984 Mercedes hit by Stephenson and was killed.

Jeffrey A. Goudy, 52, of Mount Vernon, was injured in his 2002 Honda and taken to a hospital.

Steven W. Brown, 51, of Mount Vernon was not injured in his 2013 Ford Focus.

Impairment and Lack of Seat Belt are Factors in St. Charles Parish Fatal Crash

St. Charles Parish 12/08/2015 —  – Lousiana State Police report that on Dec. 2, 2015 at approximately 9:30 pm, Louisiana State Police Troop B was notified of a fatal crash on LA 18 near LA 3142. The two-vehicle crash claimed the life of 34-year-old Travis Joseph of St. James, LA.

Preliminary investigation revealed that Joseph was driving a 2002 Ford Escort west on LA 18 and, for unknown reasons, the Ford Escort crossed into the eastbound lane of travel and struck a 2002 GMC Envoy head-on. The GMC Envoy was driven by Marshall Smith (age 55) of Ethel, LA. Smith sustained moderate injuries and was transported to St. Charles General Hospital along with a passenger from Mr. Joseph’s vehicle, Cortney Madere (age 27) of Des Allemands, LA. Joseph was pronounced dead on the scene.

Troopers suspect that Joseph was impaired at the time of the crash and a toxicology test is pending an autopsy. Smith and Madere were wearing seat belts, however, Joseph was not. The crash remains under investigation.

42% of Louisiana’s fatal crashes in 2014 were caused by alcohol impairment. Troopers witness first-hand the devastating effects that impaired driving crashes have on families across our state. Driving impaired is illegal. Louisiana State Police would like to remind citizens that having a plan in place for a safe ride home decreases fatal and injury crashes. Always remember to drive sober, never get into a vehicle with an impaired driver and always buckle up, regardless of your seating position! These few simple steps will greatly reduce your chances of being injured or killed in motor vehicle crashes.

Nicolas Porche Arrested After Manhunt Following Fatal Crash in West Baton Rouge Parish
Erwinville, LA — 12/08/2015– Lousiana State Police report that at approximately 5:00 am on November 20, 2015, Troopers from Louisiana State Police Troop A began investigating a two vehicle fatality crash that occurred at the intersection of LA Hwy 620 and LA Hwy 983 (Section Rd. at Bueche Rd.) in West Baton Rouge Parish. The crash took the life of 53-year-old Michael Holmes of Ventress, LA.

The initial investigation by State Police revealed that the crash occurred as 27-year-old Nicholas Porche was traveling westbound on LA Hwy 620 in a white 2008 GMC Sierra. At the same time, Holmes was traveling southbound on LA Hwy 983 in a red 1998 Ford F-150. As Porche approached the intersection of LA Hwy 620 and LA Hwy 983, he failed to yield at the stop sign and proceeded into the path of Holmes’ vehicle. Holmes was unable to avoid Porche’s vehicle and struck it on the left side. After impact, Porche’s vehicle began to overturn and came to rest on the northwest side of the intersection.

After the crash, Porche fled the scene into a cane field. Troopers, along with Deputies from the West Baton Rouge Sheriff’s Office and the Baton Rouge Police Department’s K-9 Officers, searched the area for Porche. The Louisiana State Police Aviation Unit responded to the scene with a helicopter to assist with the search and was able to locate Porche in the cane field approximately ½ mile from the crash scene.

Holmes was not properly restrained and sustained fatal injuries as a result of the crash. He was pronounced deceased at the scene by the West Baton Rouge Parish Coroner’s Office. Porche was properly restrained and received minor injuries in the crash but refused aid. A toxicology sample will be taken from Holmes but impairment is not suspected.

Troopers do suspect impairment on the part of Porche and he was placed under arrest at the scene. A toxicology sample will be taken from Porche for analysis. Porche was transported to the West Baton Rouge Parish Jail where he was booked with 4th Offense DWI, Vehicular Homicide, Felony Hit and Run, Operating a Vehicle Without Required Interlock Device, and Failure to Yield at a Stop Sign.

Louisiana State Troopers want to inform motorists that impaired driving continues to be a major problem throughout Louisiana. Whether impaired through alcohol, drugs, or a combination of the two, the consequences of driving while impaired remain the same. Also, all occupants of a motor vehicle must be restrained, regardless of seating position. While not all crashes are survivable, statistics show that the chances of surviving a crash rise significantly when properly restrained. Making good choices while in motor vehicles such as never driving while impaired, always ensuring every occupant is properly restrained, and avoiding all distractions can often mean the difference between life and death.

Crash Leads to Arrest of Lake Arthur Man for Vehicular Homicide

LAKE ARTHUR, LA – A Lake Arthur man was arrested last night for vehicular homicide after the vehicle he was driving crashed, resulting in the death of an unrestrained passenger.

On November 14, 2015, at 12:38 a.m., Louisiana State Police Troop D responded to a single-vehicle crash on Broadmore Road near Trailer Town Road about ½ mile north of Lake Arthur in Jefferson Davis Parish. The initial investigation revealed a 2004 Toyota pickup truck was traveling north on Broadmore Road when the driver, 29-year-old Robert Leleux II of Lake Arthur, lost control of the vehicle. Once he began to lose control, Leleux overcorrected causing the vehicle to flip.

One of the passengers, 26-year-old Brandon J. Whitlow of Lake Arthur, was ejected from the vehicle. Whitlow was not wearing a seat belt and was pronounced dead at the scene by the Jefferson Davis Parish Coroner’s Office. Another passenger in the vehicle received minor injuries.

Leleux received moderate injuries and received treatment at an area hospital. He was then booked into the Jefferson Davis Parish Jail on charges of vehicular homicide, vehicular negligent injuring, DWI, careless operation, and failure to wear a seat belt.

Impairment and excessive speed are suspected factors in the crash. A toxicology sample was obtained from Leleux and will be submitted for analysis. The crash remains under investigation.

Last night, Troop D arrested 8 people in our area for DWI including 6 at a DWI checkpoint in Calcasieu Parish. We will continue to work hard to remove impaired drivers from our roadways but ask individuals to help by doing whatever they can to prevent impaired drivers from getting behind the wheel. Friends don’t let friends drive drunk. We also encourage anyone who observes an impaired driver to dial *LSP (*577) to report the driver to Louisiana State Police or to contact local law enforcement.

Louisiana law requires vehicle occupants to be properly restrained in all seating positions, day and night. Wearing a seat belt on every trip, whether it’s short or long, is the most effective thing motorists can do to protect themselves in the event of a crash. Motorists are encouraged to always make the smart choice and to wear their seat belt.

Troop D has investigated 27 fatal crashes resulting in 29 fatalities this year.

DRUNKEN DRIVER CHARGED WITH RUNNING RED LIGHT AND CRASHING INTO ON-DUTY LASD K-9 VEHICLE AND INJURING ONE DEPUTY

WESTMINSTER, Calif. – The Orange County District Attorney announced that a drunken driver was charged and arraigned Dec. 7, 2015, for running through a red light and crashing into an on-duty Los Angeles County Sheriff’s Department (LASD) K-9 unit’s vehicle and injuring one of the deputies.

William Long Thai Nguyen, 29, Corona, is charged with one felony count of driving under the influence of alcohol and drugs causing bodily injury with one prior, one misdemeanor count of driving a motor vehicle without a valid license, an infraction for possession of marijuana while driving, and a sentencing enhancement for inflicting great bodily injury.

The defendant also has a prior misdemeanor conviction in 2013 for driving under the influence of alcohol or drugs in Orange County. If convicted, the defendant faces a maximum sentence of six years in state prison and six months in jail. Nguyen is scheduled for a pre-trial hearing on Dec. 15, 2015, at 8:30 a.m. in Department W-12, West Justice Center, Westminster.

At approximately 4:30 a.m. on Dec. 3, 2015, Nguyen is accused of consuming alcohol and methamphetamine before getting into his vehicle and driving eastbound on Slater Street in Huntington Beach. The defendant is accused of failing to stop at a red light and crashing into a LASD deputy’s K-9 unit vehicle, which was passing legally on the westbound side of the street.

Nguyen is accused of crashing into the victims’ car resulting in the deputy driving being knocked unconscious and suffering additional injuries including an abrasion above the victim’s left eye and a contusion on the victim’s right knee. Paramedics responded to the scene and the victim was transported to the University of California, Irvine Medical Center. The defendant sustained minor injuries in the crash and was transported to the Western Medical Center in Santa Ana. The deputy’s K-9 deputy dog was in the backseat of the vehicle. The investigation is ongoing as to whether the K-9 deputy suffered any injuries.

The Huntington Beach Police Department investigated this case.

Deputy District Attorney Angela Hong of the Special Prosecutions Unit is prosecuting this case.

SPARTA, NEW JERSEY — 12/10/2015 — Sparta Police report that on 12-08-2015 at approximately 11:06 PM, Cpl. Adam Carbery was travelling southbound on Route 15 when he observed a white 2014 Jeep Grand Cherokee travelling the wrong way northbound in his lane of travel.

After taking evasive action and avoiding a head-on collision with this vehicle, Cpl. Carbery turned around into the northbound lanes in order to pursue this vehicle and attempt to stop it.

The white Jeep continued northbound in the southbound lanes and before any Sparta patrol was able to catch up to the vehicle, it struck a 1998 Toyota Corolla head-on.

The operator of the Jeep Grand Cherokee, identified as 50-year-old Teresa Verbout from Sparta, suffered serious injuries to her lower extremities. She was flown to Morristown Medical Center. The driver of the Toyota Corolla, 22-year-old Robert J. Hunter 3rd from Flanders, was pronounced deceased at the scene.

Verbout has been charged criminally with Vehicular Homicide, a second-degree crime. She has also been charged with DWI. The investigation is still ongoing.

The Sparta Police were assisted by NJ State Police, the Sussex County Prosecutor’s Office and the Medical Examiner’s Office.

ROCKVILLE, MD. — 12/10/2015 — It is with profound sadness that the Department confirms that Montgomery County Police Officer Noah Leotta has succumbed to injuries he sustained when he was struck by a driver suspected of driving under the influence of drugs and alcohol on December 3.

Officer Noah Leotta, age 24, grew up in Montgomery County, was a Sherwood High School graduate, and attended Montgomery College. Officer Leotta interned with the Police Department from December 2011 to January 2013. He shadowed 1st District Officer John Romack, whose primary focus was enforcement of impaired-driving laws, and then was mentored by officers of the Alcohol Initiatives Section.

In January 2013, Officer Noah Leotta was hired as a Montgomery County Police Officer Candidate and attended the Montgomery County Police Academy as a member of Session 58. The class graduated in July 2013 and Noah was assigned to the 4th District for field training. When he completed his training, he remained in the 4th District as a 4thDistrict patrol officer.

On the night that he was struck, Officer Leotta was working as a member of the annualHoliday Alcohol Task Force. These officers devote their full time and attention to detecting alcohol-related offenses during the holiday season.

Officer Noah Leotta had a passion for serving the community and was an exemplary officer as evidenced by comments in a recent performance evaluation. His supervisor wrote, “He arrives at work early every day with a smile on his face as though he can’t wait to start the day. His enthusiasm for the job is unwavering and infectious.” His supervisor further explained, “He is humble and selfless and never seeks any recognition for himself. Officer Leotta is always willing to help out fellow officers in any way he can.”

Officer Struck by Suspected DUI Driver; Remains in Critical Condition

Dec. 4, 2015 — Last night at approximately 9:47 p.m., an on-duty Montgomery County Police officer was struck by a passing vehicle as he conducted a traffic stop.  The officer, identified as Noah Leotta, remains hospitalized with life-threatening injuries.

The on-scene investigation by Collision Reconstruction Unit detectives has revealed that Officer Leotta was conducting a traffic stop in the curb lane of northbound Rockville Pike and Edmonston Drive.  During the course of this traffic stop, a 2012 Honda CRV, traveling north on Rockville Pike, first struck Officer Leotta’s marked cruiser and then struck Officer Leotta, who was outside of his vehicle.

The driver of the CRV, identified as Luis Gustavo Reluzco, age 47, of Rolling Meadow Way in Olney, remained at the collision scene.  Luis Reluzco was taken into custody under the suspicion of driving under the influence of alcohol and drugs.  He had no reported injuries.  Due to the seriousness of Officer Leotta’s injuries, Reluzco was required by Maryland law to submit to a blood test that will be analyzed for alcohol;  those results are pending.  Collision Reconstruction Unit detectives continue to examine the circumstances of this collision and if charges are to be made, they will be placed after the completion of their investigation.

Officer Noah Leotta, age 24, has been a Montgomery County Police officer since January of 2013.   Officer Leotta is a 4th District officer who is currently working as a member of the Holiday Alcohol Task Force. The officers of the Holiday Alcohol Task Force devote their full time and attention to detecting alcohol-related offenses during the holiday season.  Before serving as a Montgomery County Police officer, Officer Leotta interned with the Department in a unit that focuses on impaired-driving enforcement.

“Noah is not only a dedicated young police officer, he is a caring person who became a cop to serve the community where he grew up.  He needs everyone’s prayers as he fights for his life,” says Chief of Police Tom Manger.

The men and women of the Department thank the community for the continued thoughts and prayers being sent to Officer Leotta.

12/11/2015 — Virginia State Police report that they investigated a single vehicle accident on the Eastern Shore, which has resulted in a fatality.

On December 9, 2015, at approximately 9:23 P.M., Virginia State Police was called to investigate a single vehicle accident involving serious life threatening injuries. The accident occurred on Route 13 (Lankford Highway) north of Tyler Lane in the County of Accomack.

A 2002 Mitsubishi Lancer, driven by Brandon O’Neil West, was traveling northbound Route 13 at a high rate of speed. The driver lost control of the vehicle, and the vehicle spun around backwards into the median that separated the north and southbound lanes of Route 13. The vehicle struck a culvert that was in the median, and upon impact, ejected the backseat passenger.

The back seat passenger, Devante Deshawn Harris, suffered major life-threatening injuries and was taken to Peninsula Regional Medical Center in Salisbury Maryland. Mr. Harris later died of his injuries at the hospital. Family members were notified.

There were four occupants inside the vehicle. All four occupants in the vehicle were not wearing their safety belts, and alcohol was involved. The right rear passenger was critically injured and flown to Norfolk Sentara Hospital. The front seat passenger also suffered major injuries.

The driver, Brandon O’Neil West, was charged with involuntary manslaughter and for driving under the influence.

A day after the death of a police officer in Maryland who was hit by a suspected drunk driver, calls are increasing for harsher driving-under-the-influence punishments in the state.

Speaking after the death of Officer Noah Leotta, 24, Montgomery County Police Chief J. Thomas Manger spoke furiously.

“The state of Maryland has some of the weakest penalties for drunk drivers, and in fact, as we look ahead to the prosecution of this case, I can tell you that whenever it’s done, there’s little chance of real justice being done,” he said.

Leotta was critically injured the evening of Dec. 3 when a driver suspected of being drunk slammed an SUV into Leotta’s cruiser and then into the officer as he pulled over another driver suspected of being drunk. The driver, Luis Gustavo Reluzco, 47, has not been charged. Results of a blood test to detect substances in his system are pending.

The maximum penalty in Maryland for killing someone while driving drunk is 10 years, but few defendants ever get that sentence, advocates say.

“Generally, when someone [driving drunk] kills someone in Maryland, they’re looking at about 18 months,” said Lisa Spicknall, state director of Mothers Against Drunk Driving. She has helped hundreds of families cope with the loss of a loved one to a drunk driver.

“We all would think that such a person would go to jail for a long time, but number one, the judges are inconsistent. Number two, the prosecutors are inconsistent in terms of seeking serious penalties. And number three, the laws just don’t provide for a long jail sentence,” Spicknall said.

“It’s a traffic offense, it’s not a criminal offense,” Rene Sandler said. She’s a criminal defense attorney and former prosecutor who also was the victim of a drunk driver.

In Virginia and D.C., penalties for fatal drunk driving carry 20 to 30 years in prison. Also, unlike in other states, Maryland does not allow victims or their families to sue a person or establishment that served a drunk driver too much alcohol.

Last year, 121 people were killed in Maryland by drunk drivers.

Short prison sentences for drunk drivers re-traumatize their families, Spicknall said.

“It sets them back to day one, to that day when they were first informed that their loved one was killed,” she said.

The MADD leader said she hopes the tragedy of Leotta’s death can spark change among lawmakers.

“Maybe this is the case that makes the senators and delegates step up and say, ‘We’re done with this and we’re tired of it and we want to see these laws,'” Spicknall said.

Advocates will return to Annapolis next year to urge lawmakers for harsher penalties.

A Naples father of four was killed and two others injured Saturday evening in a three-vehicle crash.

The Florida Highway Patrol reports James Riner II, 33, was driving on Golden Gate Boulevard when his car was struck head-on by a vehicle that was forced off the road by another vehicle.

The driver who troopers say caused the crash has been identified as Rene Miles, 40, of Naples.

He is charged with DUI manslaughter, DUI property damage, DUI personal injury, leaving the scene of a fatal crash, leaving the scene of an injury crash, and leaving the scene of a crash with property damage.

Troopers say Miles was traveling on Golden Gate Blvd. when he rear-ended another vehicle, forcing that vehicle to lose control before it struck the vehicle driven by Riner.

Riner’s passenger had minor injuries. The driver of the other vehicle sustained serious injures. Her 13-year-old passenger was uninjured.

The crash occurred around 7 p.m. near 27th Street SW.

A GoFundMe account has been set up for the family of Riner, a married father of four boys.

Right now, all lanes are back open on Golden Gate Blvd.

Riner’s van also overturned several times as a result of the crash and he was pronounced dead at North Collier Hospital Northeast. Jessica Riner suffered a broken hand and was taken to Lee Memorial Hospital.

Beauplan suffered serious injuries and was taken to Lee Memorial Hospital where she was listed in stable condition Sunday night. A passenger in her SUV, Dreamson Beauplan, 13, was not hurt.

Miles was not hurt and the FHP report said that alcohol was a factor in the crash.

The FHP also reported that Miles initially left the scene of the crash.

He was arrested and charged with DUI manslaughter, DUI personal injury, DUI property damage, leaving the scene of a crash with a fatality, leaving the scene of a crash with property damage and leaving the scene of a crash with personal injury. ....MORE

Memphis police have arrested a man accused of driving drunk and killing a passenger in another car early Sunday morning, officials said.

Jose Lopez, 27, has been charged with vehicular homicide involving intoxication, vehicular assault, DUI, public intoxication, reckless driving and other traffic offenses. He is being held in the Shelby County Jail.

According to police, this incident happened just before 2:30 a.m. at Raleigh-Millington Road and St Elmo in the Raleigh area.

 12/17/2015–  Washington State Patrol (WSP) is seeking witnesses to a two-vehicle, fatal collision that occurred on I-5/304th (Federal Way) on December 17, 2015, at approximately 1:40 am.  The victim vehicle, a 1987 Honda Civic two door, was disabled and blocking near the right lane when it was struck by a 1992 Ford Mustang.  The two female occupants in the Honda died at the scene.

Washington, D.C.  – The U.S. Marshals North Texas Fugitive Task Force is currently searching for fugitive Ethan Couch, 18, wanted for probation violation out of Tarrant County. Couch’s case made national news in 2013 when he was sentenced to 10 years probation for the vehicular manslaughter of four people. On December 11th, a warrant was issued for Couch’s arrest.

U.S. Marshals are working in conjunction with state, local, and federal agencies in the apprehension of Couch. A reward of up to five thousand dollars is being offered for information that leads to the arrest of Couch.

U.S. Marshals task forces combine the efforts of federal, state, and local law enforcement agencies to locate and arrest the most dangerous fugitives. In 2014, U.S. Marshals arrested more than 104,000 fugitives.

Unrestrained 21 Year Old Passenger Killed in Ascension Parish Crash

Gonzales – Shortly after 9:30 pm on May 22, 2015, Troopers from Louisiana State Police Troop A began investigating a single vehicle fatality crash that occurred on US Hwy 61 south of LA Hwy 621 in Ascension Parish. The crash took the life of 21-year-old Dylan T. Martin of Gonzales, LA.

The initial investigation by State Police revealed that the crash occurred as 24-year-old Jason L. Williams of Gonzales was driving a 2005 Suzuki Forenza southbound on US Hwy 61. For unknown reasons, the Suzuki ran off the roadway to the right. After leaving the roadway, the vehicle struck a culvert causing it to roll several times before coming to rest on its roof.

Martin (the front seat passenger) was unrestrained at the time of the crash and was partially ejected from the vehicle. He suffered fatal injuries as a result. Williams was properly restrained at the time of the crash. He was transported from the scene to Our Lady of the Lake Regional Medical Center with minor injuries. Impairment on his part is a possible factor, and a toxicology sample was taken from him and submitted to the Louisiana State Police Crime Lab for analysis. Williams was charged with LRS 32:58 – Careless operation. This crash remains under investigation, and further charges may be pending.

Troopers wish to remind motorists of the importance of seat belt use, especially during a busy Memorial Day holiday. Louisiana State Police is currently participating in the nationwide Click It or Ticket initiative and will be proactively seeking unrestrained drivers and their passengers. Louisiana law requires all occupants to be properly restrained regardless of seating position. While not all crashes are survivable, occupants who are properly restrained are much less likely to be seriously injured or killed in the event of a crash.

LAKEPORT, CALIF. — 12/22/2015 — According to the Lakeport Police, at approximately 7:20 AM on Dec. 21, 2015, Lake County Central Dispatch received an emergency call from the California Highway Patrol Communications Center in Ukiah reporting that they received a call from a Lakeport man who stated that his wife told him she was going to kill herself by running her car into another vehicle. The woman identified as 37-year-old Dana Marie Lindsey was reported to be driving a 1997 gold Volkswagon convertible in the area of Lakeport Blvd and Hwy 29. Lakeport Police Department, California Highway Patrol, and Lake County Sheriff’s Office Units responded to the scene.

At some point, officers received information that Lindsey’s husband had been hanging onto the vehicle and fell or jumped off.

Responding units attempted to stop Lindsey as she traveled southbound on Hwy 29 from Lakeport Blvd., but she evaded them and made two attempts to drive her vehicle into another vehicle traveling north bound, but they were able to avoid a collision.

Lindsey continued to travel southbound and intentionally collided with a 2009 Toyota Corolla, just south of the intersection of Highways 29 & 175 Hopland. A 24-year-old male passenger in the victim vehicle was seriously injured and transported via ambulance to Sutter Lakeside Hospital and is expected to be transferred to Santa Rosa Memorial Hospital for further treatment. The 28-year-old female driver of the victim vehicle received minor injuries and was later transported by private party to Sutter Lakeside where she was treated and released.

Lindsey, who officers believed to be under the influence of methamphetamine and MDMA, suffered minor injuries and was also transported by ambulance to Sutter Lakeside where she was treated and released at approximately 4:00 PM. At that time, LPD officers responded to the hospital and arrested her for attempted homicide, two counts of assault with a deadly weapon, felony evading a peace officer, felony driving under the influence causing great bodily injury and misdemeanor drunk driving. She was transported to the Lake County Correctional Facility where she was booked and is being held on bail of $1,000,000.

The CHP is also working with LPD on this investigation that is ongoing. Lindsey’s vehicle was seized as evidence and is being held pending further processing.
